Without automation, every sale is a small piece of manual work. A payment notification arrives. You open the panel. You create the user. You set the right package and expiry. You email the customer their credentials. Five minutes, maybe less.
Five minutes is nothing at ten customers. At three hundred customers renewing on three hundred different dates it is a full-time job that pays nothing and produces an outage every time you are asleep when someone buys. Most small VPN operations do not fail because the product was bad. They stall because the founder became the provisioning system.

The customer receives their service
They sign in to your app with those credentials and connect.
Renewal renews the account
A paid renewal invoice extends the account. An unpaid one lets it lapse on your rules.
Reseller automation, where supported
The same principle applied one level up. Instead of you allocating credits to resellers by hand, a reseller buys a credit package in your store and the module tops up their panel account.

WHMCS is licensed separately. WHMCS is a commercial product from WHMCS Limited with its own licence fee. The $199 module runs inside a WHMCS installation you own — it is not a billing system in itself.
When to buy it
Not necessarily first. If you are pre-launch with no customers, the panel, an app and a server get you selling; creating your first twenty accounts by hand is fine and teaches you how the packages behave. Buy the module at the point where provisioning has become the thing you do instead of the thing you avoid — usually somewhere between fifty and a hundred active users, or the first time a renewal is missed because you were asleep.

Do I need WHMCS already?

The account follows the expiry rules you set in the panel. The point of the module is that this happens on its own instead of you reconciling a payment list against a user list by hand.
